I understand and agree that work will not begin on my Personalized Clinician Page until the Page Creation Fee of $250.00 is tendered and received by PayPal Services. I understand and agree that a turnaround time of 10 business days (two calendar weeks) is the standard time frame past the point of payment for the developer to convert my personal information and publish my page. I further understand that time may be shorter or longer depending on the backlog of clients, but communication to the Account Email address provided in this form will be sent in a timely manner if the turnaround time of 10 business days can not be met for any reason. I understand and agree that MusicClinicians.org (a subsidiary of Sprankle Design, LLC) will securely and continuously host my likeness and intellectual property that I freely submit to this page, and any infringement or unauthorized usage of my intellectual property by a third party must be contested and resolved by you, the owner of said property. Further, I understand and agree that submitting my likeness and intellectual property to MusicClinicians.org does not transfer ownership to the subsidiary – all materials presented on your Personalized Clinician Page is your legal property to share, network, and promote as you see fit. I understand and agree that every 365 days from the received Page Creation Fee, an annual invoice of $100.00 will be sent via PayPal Services for the renewal and continued hosting, domain, and editing services of my page (including a courtesy reminder via email 14 days prior to the annual charge). I understand and agree that my account and page can be canceled and deleted at any time – incurring no further charges from the date of cancelation – but no backdated refunds will be issued.
